Program Application

This application is for young adult cancer fighters and survivors. Completion of this form is mandatory for all participants and must be returned prior to acceptance for a Send It Foundation outdoor program. This form is for us to get to know our participants. It is important to note that the information provided will not determine whether or not you will be placed on a trip. All of our programs operate on a first-come, first-served basis.
