Program Application

This application is for young adult cancer fighters and survivors. Completion of this form is mandatory for all participants and must be returned prior to acceptance for a Send It Foundation outdoor program. This form is for us to get to know our participants. It is important to note that the information provided will not determine whether or not you will be placed on a trip. All of our programs operate on a first-come, first-served basis.

Once we receive your application, a program coordinator will follow up with next steps. We will do our best to place you on your desired program.

In addition to this application, all participants must complete our Medical Form, which will be filled out by their primary oncologist/physician, as well as our Liability Form, Model Release, and Cancellation Policy. These forms will be shared with participants upon receipt of their application.
